PASSPORT SERVICES TO BE RENDERED BY PASSPORT SECTION

(The Passport/Consular fees have been changed w.e.f. 01st October, 2012. Please look at the relevant Section of the website for the further details.) 


(IT MAY BE NOTED THAT THE PASSPORT SECTION WOULD BE RENDERING ALL SERVICES, WHICH ARE TO BE RENDERED BY OUTSOURCING AGENTS TILL SUCH A TIME THE OUTSOURCING AGENT’S JEDDAH OFFICE IS NOT GEARED TO TAKE UP ITS ENTIRE GAMUT OF SERVICES)
 

 
CASES WHRE A PASSPORT BOOKLET IS TO BE ISSUED
 
 

  • Full Validity of a Passport for an applicant below 15 years is : 5 years
  • Full Validity of a Passport for an applicant above 15 years is : 10 years
  • Sworn Affidavits or Passport application form or any other declaration made by the minor applicants will have to be countersigned by his/her parent or legal guardian
  • An applicant applying for a passport booklet under TATKAAL Scheme will have to pay SR.565/- in addition to the other applicable fees.  Passport applied under TATKAAL Scheme are normally given the SAME DAY.

 
PLEASE NOTE: Passport issuance cases where PISON or PAC check reveals the existing passport as IMPOUNDED/REVOKED, applicant falling under Prior Approval Category (PAC), PVR Adverse, PVR Incomplete require thorough scrutiny and processing of these cases need great deal of efforts on the part of dealing hand as he has to do protracted correspondence with the PIAs to obtain clearance for such cases. 
 
 

  • Re-issue of Passport Booklet cases Where Passport expired before three years

    • Form No.10 (Late Renewal Form) – giving reason for not submitting the passport for renewal

  • Changing First Name and the Surname altogether

  • When the passport holder applies for correction of date of birth and place of birth
    • EAP-1 (Form No.1)
    • Form No.9 (Sworn Affidavit giving reasons for reflection of wrong date of birth in the present passport and the why does he wants to change/correct the date of birth now.  He/She should provide sufficient proof for proving the correct date of birth)
    • Copy of duly attested birth Certificate (original is to be seen) – Original Birth Certificate should be attested by the G.A.D. or Home Deptt. of the concerned State
    • Copy of duly attested Secondary School Certificate (original is to be seen) – Original Secondary School Certificate should be attested by the competent authorities in India (authorized to attest the educational documents for overseas submission purpose)
    • If the applicant is an illiterate or non-matriculate an Sworn Affidavit to this effect needs to be obtained from the applicant in stead of the Secondary School Certificate.
    • 3 Photographs
    • Passport Copy
    • Iqama Copy
    • If Iqama is not valid then an undertaking from Sponsor is required, stating that the sponsor undertakes to renew his employee’s Iqama once passport is ready (in that case a 2 YEAR SVP will only be issued).

  • Birth Registration of a new born child as Indian citizen and issuance of a separate Indian Passport to him/her  where one of the parent is a foreign national (within one year of the birth of the child)
    • EAP-1 (Form No.1)
    • Form No.22  (Registration of Birth Form)
    • Form No.24 (Affidavit by the Indian parent)
    • 3 Photographs of the child
    • Passport Copy of Indian parent (should have name of child’s father/mother written in his/her passport, if not, the he/she should also apply for a new passport booklet and get his/her spouse name added in the passport)
    • Passport copy of the foreign parent
    • Iqama Copy of both the parents
    • Copy of Marriage Certificate of parents (Original to be seen)
    • Copy of Registration of Marriage of the Parents with the Registrar of Marriages in India or with the Marriage Officer at any of the Indian Missions or Posts abroad (If not registered so far, the marriage needs to be registered first and then only the BRSP service will be provided).
    • Original Passport of the Indian Parent (BR entries will be made in it)
    • Fee : SR.282 (188 + 94) + 8
    • In case of Jumbo Passport : SR.376 + 94+ 8

  • Birth Registration of a new born child as Indian citizen and issuance of a separate Indian Passport to him/her  where one of the parent is a foreign national (after one year of the birth of the child) – the case needs to be referred to MHA/MEA
    • EAP-1 (Form No.1)
    • Form No.22  (Registration of Birth Form)
    • Form No.24 (Affidavit by the Indian parent)
    • Applicable form for nationality case for new born child
    • 3 Photographs of the child
    • Passport Copy of Indian parent (should have name of child’s father/mother written in his/her passport, if not, the he/she should also apply for a new passport booklet and get his/her spouse name added in the passport)
    • Passport copy of the foreign parent
    • Iqama Copy of both the parents
    • Copy of Marriage Certificate of parents (Original to be seen)
    • Copy of Registration of Marriage of the Parents with the Registrar of Marriages in India or with the Marriage Officer at any of the Indian Missions or Posts abroad (If not registered so far, the marriage needs to be registered first and then only the BRSP service will be provided).
    • Original Passport of the Indian Parent (BR entries will be made in it)

  • Birth Registration of a new born child as Indian citizen and issuance of a separate Indian Passport to him/her  where both the parents are Indian nationals, however, one of the parents is not available and his consent on the form is not obtained (mostly such cases belong to the ladies working here as nurses and their husbands are either in some other country or in India – working and residing there) :
    • EAP-1 (Form No.1)
    • Form No.22  (Registration of Birth Form)
    • Form No.23 (Affidavit by the available parent)
    • 3 Photographs of the child
    • Passport Copy of available parent (should have name of child’s father/mother written in his/her passport, if not, the he/she should also apply for a new passport booklet and get his/her spouse name added in the passport)
    • Passport copy of the parent whose consent has not been taken
    • Iqama Copy the available parent
    • Original Passport of the available Parent (BR entries will be made in it)
    • Fee : SR.282 (188 + 94) + 94+8
    • In case of Jumbo Passport : SR.376 + 94+ 8

  • Passport booklet under TATKAAL Scheme (SR.565/- to be charged in addition to the other applicable fees):
    • EAP-1 (Form No.1)

    • 3 sets of PP Forms – with correct particulars
    • 3 Sets of forms in respect of double/multiple passport cases
    • 9 Photographs
    • Passport Copy of all passports held by the applicant
    • All original passport booklets held by the applicant
    • Iqama Copy
    • An explanation from the applicant giving circumstances under which he obtained these passports
    • Copy of supporting documents providing correct personal particulars of the applicant (originals need to be seen)
    • Fee : SR.282 + 475+94 + 8

    • THE APPLICANT NEEDS TO BE THOROUGHLY INTERVIEWED; CASE NEEDS TO BE REFERRED TO THE CONCERNED PIAs FOR VERIFYING THE PERSONLA PARTICUALRS AND PHOTOGRAPHS OF THE APPLICANT AND A PASSPORT NEEDS TO BE ISSUED AFTER RECEIVING CLEAR PVR AND CLEARANCE FROM THE CONCERNED PIAs.  MINISTRY ALSO NEEDS TO BE INFORMED. 

 
 CONSULAR SERVICES TO BE RENDERED BY PASSPORT SECTION
  

  • Birth Certificate after change in the name, parents name, etc.  – REVISED B.C.
  • EAP-II Form (Form No.2) –  request for a birth certificate with desired correction
  • Proof substantiating the change needs to be submitted. – Consular Officer has to approve the same
  • One Photograph
  • Passport copy of the person whose Birth Certificate is required
  • Passport copy of the person who is submitting the request, in case the person is different
  • Iqama copy of the person who submits the application
  • Fee :  SR. 94 + 8

  • BIRTH RECORDS AS REGISTERED WITH THE CONSULATE NEEDS TO BE CHECKED AND VERIFIED BEFORF A REVISED B.C. IS ISSUED.

 
 ATTESTATION SERVICES TO BE RENDERED BY PASSPORT SECTION
 
 

  • Attestation of the Power of Attorney (POA)
  • Power of Attorney document to be attested should be provided with Original + one photo copy
  • Two photographs (pasted one on the original and one on the copy)
  • Passport copy of the executant of the POA (who is signing the POA)
  • Iqama copy of the executant
  • Fee : SR.80 + 8 (for POA having financial liability on the part of executant)
  • Fee : SR.40 + 8 (for POA not having any financial liability on the part of executant)
  • APPLICANT SHOULD BE PRESENT IN PERSON AND SIGN THE POA

  • Attestation of the Sponsorship Declaration (SD)
  • Sponsorship Declaration to be attested should be provided with Original + one photo copy

  •  FORM 8    Change of Profession Sworn Affidavit (in English and Arabic)
    • Affidavit to be attested should be submitted in DUPLICATE (2 sets)
    • Passport copy of the Deponent (who signing the SA under oath)
    • Iqama copy of the Deponent
    • Proof (degree/diplomat/experience certificate) to justify the knowledge of profession which the applicant desires to change now

    • APPLICANT SHOULD BE PRESENT IN PERSON AND SIGN THE SA

  •  FORM 11  Life Certificate (LC)
    • should be submitted in DUPLICATE (2 sets) – signature of the applicant to be taken in presence of the Consular Officer / Counter Official and his/her identity should be verified through an original photo ID

    • Fee : NO FEE / GRATIS
    • APPLICANT SHOULD BE PRESENT IN PERSON AND SIGN THE LC

  •  FORM 14  Marriage Joint Sworn Affidavit (for Indian nationals only)
    • should be submitted in DUPLICATE (2 sets) – signature of the husband and wife to be taken in presence of the Consular Officer / Counter Official and their identity should be verified through an original photo ID
    • Passport copy of the couple
    • Iqama copy of the couple
    • Copy of the Marriage Certificate (if available), if not, then a joint photograph of the couple needs to be pasted on the Jt. SA (on both original and copy)
    • Fee : 40 + 8
    • BOTH HUSBAND AND WIFE SHOULD BE PRESENT IN PERSON AND SIGN THE SA

  •  FORM 15  Marriage Sworn Affidavit
    • should be submitted in DUPLICATE (2 sets) – signature of deponent to be taken in presence of the Consular Officer / Counter Official and his/her identity should be verified through an original photo ID

    • Iqama copy of the deponent
    • Copy of the Marriage Certificate
    • Fee : 40 + 8

  •  FORM 16  NOC by BOTH parents when child already has a passport
    • Affidavit to be attested should be submitted in DUPLICATE (2 sets) - signature of both the parents to be taken in presence of the Consular Officer / Counter Official and their identity should be verified through an original photo ID
    • Passport copy/copies of the child/children for whom the NOC is being given
    • Passport copy of both the parents
    • Iqama copy of both the parents
    • Fee : SR.40 + 8

  •  FORM 17  NOC by parent when child is applying for a passport first time
    • Affidavit to be attested should be submitted in DUPLICATE (2 sets)
    • Copy of birth certificate of the child/children for whom the NOC is being given

    • Iqama copy of both the parent signing the NOC
    • The parent who signs the NOC should be present in person and sign before the Consular Officer or the Consular Official

  •  FORM 18  NOC by parent when child already has a passport
    • Affidavit to be attested should be submitted in DUPLICATE (2 sets)
    • Copy of Passport of the child/children for whom the NOC is being given

    • Iqama copy of both the parent signing the NOC
    • The parent who signs the NOC should be present in person and sign before the Consular Officer or the Consular Official

  •  FORM 19  NOC by BOTH parents when child is applying for a passport first time
    • Affidavit to be attested should be submitted in DUPLICATE (2 sets) - signature of both the parents to be taken in presence of the Consular Officer / Counter Official and their identity should be verified through an original photo ID
    • Copy of the Birth Certificate(s) of the child/children for whom the NOC is being given
    • Passport copy of both the parents
    • Iqama copy of both the parents
    • Fee : SR.40 + 8

  •  FORM 20  NOC by husband for his wife applying for a passport in India
    • Affidavit to be attested should be submitted in DUPLICATE (2 sets)
    • Copy of marriage certificate
    • Passport copy of the husband signing the NOC
    • Iqama copy of both the husband signing the NOC
    • The deponent should be present in person and sign before the Consular Officer or the Consular Official
    • Fee : SR.40 + 8

  •  FORM 25  Sponsorship Declaration (SD) Form
    •  SD to be attested should be submitted in DUPLICATE (2 sets)
    • Salary Certificate of the applicant duly attested by the Chamber of Commerce
    • Copy of Passport(s) of the person(s) who are being sponsored
    • Copy of the visa obtained (if available)
    • Passport copy of the applicant signing the SD
    • Iqama copy of the applicant signing the SD
    • The applicant should be present in person and sign before the Consular Officer or the Consular Official
    • Fee : SR.40 + 8

  • Attestation of the Driving Licenses issued in India
  • Original Driving License
  • 2 copies of the Driving License with Arabic Translation
  • Passport copy of the person whom the Driving License belong to
  • Passport copy of the person who actually bring the DL (in case the DL holder himself does not bring the same)
  • Iqama copy of the DL holder or the one who actually submits it in stead of the DL holder
  • Fee : SR.40 + 8

  • TO BE ATTESTD ONLY WHEN PRIMA FACIE THE ORIGINAL LICENSE IS FOUND TO BE A GENUINE ONE AND ITS PARTICULARS MATCHES WITH THE DL HOLDER’s INDIAN PASSPORT.

 
 

  • Attestation of the COMMERCIAL documents or POA by an individual in his/her capacity as an office bearer of a Company/Organization/Firm and not in his personal capacity
  • The Commercial document to be attested should be provided with Original + one photo copy
  • Two photographs (pasted one on the original and one on the copy), if required by the concerned authorities where the document is to be used.
  • Passport copy /Identity Card with English translation (in case of a Saudi national, not possessing a passport) of the signatory of the document
  • Iqama copy of the signatory (in case the Signatory is a non-Saudi)
  • The document should be scrutinized by Commercial Section and should be submitted with a note from the Consul (Commercial) addressed to the Consular Officer
  • The signatory should be present for identification and signature verification and if he/she is not present himself, his signature and photograph should be attested by the Chamber of Commerce on the document itself.
  • If the signatory is not present and represented by his/her authorized PRO, the PRO should also attach his/her passport copy or ID and Iqama copy (if applicable)
  • Fee : SR.200 + 8 for each document / each signature put by the Consular Officer

EMERGENCY CERTIFICATE SERVICES TO BE RENDERED BY PASSPORT SECTION
 
 

  • EC application received at the CGI Counter
  • Form No.3
  • FOUR Photographs
  • Passport Copy
  • Other supporting proof like Election ID, etc.
  • A separate letter explaining reason for requesting for an EC
  • Fee:  SR.57 +8
  • [A well charted out drill needs to be followed in such cases and a DRIL entry  needs to be made in the PISON about loss of passport of the applicant, before issuing an EC to him/her and such cases needs to be routed through Consul (CW) and Consul (Cons.)]

  • EC application received at the JAIL
  • Form No.4
  • FOUR Photographs
  • Passport Copy
  • Other supporting proof like Election ID, etc.
  • NO FEE (are issued on GRATIS basis)
  • [A well charted out drill needs to be followed in such cases and a DRIL entry needs to be made in the PISON about loss of passport of the applicant, before issuing an EC to him/her]

OTHER SERVICES TO BE RENDERED BY PASSPORT SECTION
 
 

  • Applications for Overseas Citizenship of India (OCI) Card from person of Indian origin
  • The applicant needs to apply online for an OCI (MOIA Website)
  • A file number and an application thus generated will be printed by the applicant; duly signed and submitted at the CGI Counter with the following documents:-
    • Documents providing proof of Indian nationality of the applicant himself/herself  or his/her parent/parents or his grand parent/parents
    • Copy of Foreign Passport being held by the applicant
    • Copy of his Iqama
    • Any other secondary proof held by him/her which may help him/her in making his case strong for an OCI
    • FOUR photographs
  • Fee :  SR. 1,035+8 for non-PIO Card Holder
  • Fee : SR. 95+8- for PIO Card Holder
  • Straightforward cases will be recommended for an OCI forthwith.  However, cases where no concrete proof of him/her being having Indian lineage are provided or secondary proof are provided by the applicant, will be referred to MOIA for approval.

  • Renunciation of Indian Citizenship Cases and Surrender of Indian Passports
  • The applicant needs to apply for the same in the prescribed form (available at Help Desk and at Policy Desk)
  • One Indian national has to vouch for the applicant in the form and sign thereof and he/she has to attach copy of his/her Indian Passport and Iqama
  • Original Indian Passport of the Applicant and one set of copy of the surrendered passport
  • Copy of the foreign passport(s) being held by the applicant
  • Fee & Penalty (if applicable) :  To be calculated by the Dealing Hand (Policy Seat) on the basis of the time gap between obtaining the foreign citizenship and the surrender and also whether the Indian passport was utilized by the applicant even after obtaining the foreign passport.
  • After charging the fee and penalty (if applicable), the Indian Passport needs to be cancelled and kept in the file (however, in case, the applicant request for the Indian Passport to be returned then a written request needs to be take from him and copy of all pages of the Indian Passport needs to be taken for the file before handing over the passport to the applicant)
  • A surrender Certificate needs to be issued.
